Purchase Agreement - 100% Satisfaction Guarantee

1. 100% Satisfaction Guaranteed: If I am not completely satisfied with any product, ITV Ventures™ will replace it without charge or provide me credit for the purchase price (less shipping and handling). I am entitled to a refund of the purchase price (less S & H) if I return the unused portion within 30 days with a Return Merchandise Authorization (RMA) number written on the exterior of the package. I can request an RMA by calling Customer Service at 1-800-942-2443.

2. Order Assurance Program: I may qualify for discounts up to 25% off the regular retail price on products I place on the Order Assurance Program. I agree to receive a fresh supply of my chosen Order Assurance Program product(s) every 30 or 90 days, depending on the product and quantity ordered. I can modify my Order Assurance Program account at any time.

3. Payment Authorization: I authorize ITV Ventures™ to withdraw payment equal to only the price in accordance with my customer status plus shipping and applicable sales tax from the credit card or bank account above. I agree to pay a $10.00 service fee if my check or charge is dishonored for any reason

4. Cancellation: I may cancel my Order Assurance Program at any time with no minimum purchase obligation. Cancellations must be made two (2) weeks prior to the next scheduled shipment date. I understand that by canceling, I am subject to the full retail prices and will no longer qualify to receive discounts up to 25% off.

5. Email Notifications: I consent to ITV Ventures™ emailing me regarding its products, services, income opportunity program and other topics that may be of interest or benefit to me.

6. Sales Tax Sales Tax will Be Added by ITV Where Required By Law.

7. Shipping and Handling It has been detemined by ITV's leagal department that books and ingestibles can not be shipped together and are shipped seperatly with seperate shipping and handling charges. The shopping cart for Obesity-Protocol.com does not show both shipping charges when ordering an injestible with a book.
